Building on its long-standing commitment to improve lives and communities, the Elevance Health Foundation recently launched a new initiative: Community Action Leadership, bringing together external thought leaders from across a range of disciplines. Through their collective expertise and resources, the Foundation is addressing large-scale community health needs and incentivizing action for change. The inaugural Community Action Leadership challenge focuses on patient safety. The Patient Safety Prize is seeking groundbreaking proposals for pioneering solutions in three key areas: (1) Empowering Health Literacy for Safer Patient Care, (2) Innovating to Eliminate Medication Errors, and (3) Promoting Fall-Free Futures.

Strong proposals will include community-informed solutions with demonstrated impact, have a focus on vulnerable populations, and will meet the four criteria outlined in the scoring rubric: ‍Impactful, Innovative, Measurable, and Sustainable.

PI Eligibility: Individuals with the skills, knowledge, and resources necessary to carry out the proposed research as a Principal Investigator (PI) are invited to work with their organizations to develop an application. Individuals from underrepresented groups as well as individuals with disabilities are always encouraged to apply.  

Institutional Limit: Institutions are limited to one proposal as lead applicant.

Review CriteriaConcept papers will be evaluated according to the criteria outlined in the solicitation.

Application Information: Interested PIs should submit a CV, Current & Pending, executive summary (up to 75 words), and a concept paper (up to 2 pages) describing their proposed project. The concept paper should include:

  • Challenge Statement (up to 150 words): Illustrate your understanding of the patient safety problem you are committed to solving as it relates to your chosen solution category. Identify real and potential challenges and describe specific needs that your solution will address.
  • ‍Local Conditions & Existing Landscape (up to 150 words): Describe the local conditions in and around where you intend to implement your solution, other efforts aimed at addressing this problem and, if applicable, other organizations or projects that are most similar to your work.
  • Solution Description (up to 300 words): Describe your solution and the ways in which it addresses patient safety, the relevant solution category, and your Challenge Statement. Address the project stage for your proposed solution. Provide an overview of key stakeholders and introduce the measurable impact and benefits to these communities.
